What if participants don't have any ideas?

This can happen, some images may not be inspiring for some people. If, after trying to bounce back, you still can't, then move on to another image.

Isn't that a bit random? Don't we run the risk of "missing out" on interesting ideas?

It's true that there's an element of chance in the choice of images (the initial choice from the image bank, and the choice of those you present). But if you take care to present images from very different fields, and if you systematically force associations, you won't run much of a risk, at least no more so than in brainstorming....
