ABSTRACT
Accidents, incidents, crises: severe events occur regularly within industrial organizations. Accidents are not solely attributable to human error (the "human factor") and/or technical failure. Generally, accidents announce" themselves through a certain number of deep dysfunctions in the organization which increase all along the incubation period preceeding the catastrophy. The organizational analysis is based upon the in-depth examination, within the thickness of the context, of the multiple "vertical" interactions (between management, expertise and fielsd staff) and "horizontal" ones (between the various entities which constitute the organization).

INTRODUCTION
Organizational analysis, and the associated organizational paradigm, offer a relevant and effective alternative to the industrial safety analysis methods used to date.

It is accompanied by a dynamic historical perspective on developments – safety deterioration and improvement –.
