5. Exceptional and accidental causes
When losses are the result of exceptional or accidental external actions, it is generally difficult to distinguish between the real or supposed causes, and between the role of actions and that of design. The question of whether exceptional climatic or accidental actions exceed regulatory design loads is often the subject of controversy.
