3. Illustrating the method: the Challenger case

This section presents the test carried out on the matrix calculation model for the implementation of the feared events evaluation step. It is based on feedback from the Challenger accident (explosion of an American space shuttle on January 28, 1986, killing 7 people). Analysis of the accident enabled us to establish the various hazard processes that led to the accident. The system taken into consideration is that of NASA. The hazard processes were designed to be consistent with the overall risk analysis approach presented above.

In the end, we obtained 23 hazard processes (see table 6 ).
