4. Registration forms

The essential traceability of the maintenance function's activities calls for the creation of a set of documents, in whatever form, to record the data required to establish the activity management dashboards in line with the maintenance policy applied. The set of parameters required to calculate the maintenance indicators making up these dashboards determines the nature and content of the recording forms. The sources (production/users, warehouse, maintenance, etc.) and the information system in use (CMMS or not, etc.) determine the number of forms. However, whatever the information system and its performance, a minimum of three forms is required:

• intervention request (work order or work request...) ;

• intervention report ;

• store checkout.
