In the 1990s, a famous report, "The Chaos Report", published by the Standish Group, established that the failure rate observed in the United States was in the order of 30 to 40%, depending on the size of the company; that around 50% of projects required very serious ablations to see the light of day, with the added bonus of increased costs and delays; and that only 10 to 15% were completed in line with the initial objectives.
