After this review of various aspects, we can practically recommend that, for better control of risks where human and organizational factors are not the sad inevitability, we take care to :

  • the operators' ability to understand and evaluate the rules and procedures adapted to their skills;

  • operators' ability to anticipate, and each player's visibility of what is happening upstream, downstream, and around its perimeter;

  • reaction capacity, reserves, organizational flexibility, confidence in the extraordinary capacity of human beings to adapt;

  • learning (individual and organizational), sustainability of hard-learned lessons;

  • participation: those who will take part in the performance (and possibly in accidents) must...
