6. How do you quantify a fault tree?

In hazard studies, fault trees mainly show :

  • operating equipment failures (pumps, compressors, temperature sensors, etc.);

  • failure of safety-related devices (high-level sensor, temperature sensor, valves, etc.);

  • human error due to non-compliance with procedures, errors during maintenance operations, operating errors, etc. ;

  • degradation of the mechanical properties of materials (corrosion, cracking, etc.);

  • natural hazards (landslides, lightning strikes, etc.);

  • domino effects (projectile impact, overpressure, heat flux);

  • common events (filling phase, hose connection operation, etc.).
