1. What is "5 Why?"

The "5 Whys" is a problem-solving tool. It helps identify the root causes of a problem. By repeatedly asking the question "Why?" to the problem, the layers of symptoms leading to the root causes are peeled away one by one. Although the method is called the "5 Whys", you may need to ask yourself the "Why?" question less than five times or more than five times, depending on the problem.

1.1 Method

  • Clearly state the situation, the phenomenon. You can easily identify it by asking the question "What's going on?

  • State the problem by asking "Why did this happen?

  • The answer to the question from the previous step is the direct cause of the problem. It becomes the new problem...
