4.1 BP's safety culture

The CSB's initial findings, together with the occurrence of other incidents at the site during its investigation, prompted it to ask the BP Group for a specific audit of the safety culture, both within the Group's five refineries in the USA and at Group level. An 11-member independent commission was formed, headed by J. Baker, former US Secretary of State.

• To carry out its analysis, the Baker Commission :

  • visited the five American refineries and interviewed staff at all levels of the hierarchy;

  • held public meetings in all the communities where the refineries are located;
