3. Conclusion

Failure Reporting, Analysis and Corrective Action System Methods (FRACAS – Failure Reporting Analysis Corrective Action System Methods) were originally developed by the US military in the 1980s to correct failures in their weapons systems, in order to guarantee their reliability and maintainability. Analyses of the various approaches developed in a wide range of sectors show that they all have their origins in problem-solving and total quality management methods, mainly from the Anglo-Saxon world and the quality methods developed in Japan. They involve a number of key stages: defining the problem, collecting data, analyzing it to find the root cause, defining and validating corrective actions, and verifying their effectiveness.
