5. D4 – Searching for and identifying root causes

Once the problem has been contained with effective immediate action, you can take a step back to analyze the exact cause of the malfunction.

5.1 The activities

  • Search for root causes at a first level – technical – and along two axes: the causes of the appearance of the proven problem, and the causes of the non-detection of the proven problem.

  • Search for root causes at a second level – systemic – and along two axes with, respectively, the causes of the potential problem's appearance, and the causes of the potential problem's non-detection.
