7. Identify hazards related to the human factor and outside contributors
Because an operation, particularly an industrial one, does not operate autonomously, it requires or involves human intervention (site personnel, outside contributors, even visitors).
The human factor is the human contribution involved in an event. This concept is used to study the interaction of human behavior with its environment. It also concerns the study of the reasons leading to human error. Examples include errors in machine operation, programming and product handling. These errors can lead to dangerous situations, such as those mentioned in the previous chapters:
a spill of a hazardous substance (toxic, flammable, etc.);
device malfunction (due to collision, shock, mishandling, incorrect sequencing, etc.).
