3. Multi-cause problems
In the majority of cases, the problems encountered will not be limited to a single cause. The "5 Whys" process remains the same. However, each time a problem has several causes, the causes must be dealt with one by one, in order to get back to the root causes. The result is a diagram that can be formed in Excel.
In the previous example, we could have identified two causes for the question "Why was the cold room not fed? So, the analysis continues in each branch of the analysis.
