The role of feedback in risk management

The feedback approach is a "reactive" approach to the signals emitted by installations in the various phases of operation: start-up, operation, shutdown... Its aim is to systematically take note of any malfunction detected in relation to "normal conditions" and to draw out all possible lessons to increase knowledge of the system in operation. It is a response to the problem of the "intransparency" of complex systems, for which it is not possible, a priori, to address or describe the whole combinatorial range of possible situations and their evolution.

As accidental events are, fortunately, few and far between, it's easy to see why we need to look for events other than accidents affecting people or installations.

Incidents, deviations and deviations from nominal conditions and parameters must also be analyzed.
