1. Why?
Errare humanum est, perseverare diabolicum. A mistake is forgivable, repeating it is not. This widely-held belief means that we need to be aware of our mistakes, analyze them and find ways to avoid repeating them. It's all about feedback!
Any risk study requires data. This data can only come from our own experience, or from the experience of others. Some data, such as reliability rates, can be measured. Such measurements are more time-consuming and difficult to carry out than length or mass measurements, and require specific organization. Feedback provides the data!
An organization is measured by its performance. The measurement of certain performances (reliability, safety, etc.) can be achieved through feedback!
Safety management is generally based on knowledge of the risks involved, and motivation to reduce them...
