7. Why does an MMR fail to perform its function?
The "Materialization" representation of barrier failure adapted from the SCM model (Swiss Cheese Model) (see Bibliography - Human Error, Models and Management) shows that, despite all the prevention and mitigation MMRs that may exist, there is no such thing as "zero risk". MMRs can reduce risk, but they do not eliminate it.
