7. Define risk reduction measures

7.1 Action on causes

Dealing with the causes of a function's failure most often comes down to dealing with the failures of the functions on which it depends, working backwards to the elementary functions or components.

Example of a guiding system failure due to wear :

  • Cause 1: faulty lubrication system; the FMEA for this system will then have to be carried out;

  • Cause 2: nature of materials; the design will have to be reworked to change materials;

  • Cause 3: oil change intervals not respected; a FMEA may be required for the maintenance process.
