6. How do you rate failure modes?

  • Use your customer's quotation grid, if available. If necessary, use a standardized grid or your own. Grids are also available in the FMEA study repositories (PFMEA from the Automotive Industry and Action Group).

  • For each failure mode, rate its Severity (S). Refer to internal and external effects for this assessment. The higher the value, the greater the effects for the user.

  • Rate the occurrence (O), or probability of this failure mode. The higher the value, the greater the probability of occurrence.

  • Rate the probability of non-detection (D). The higher the value, the less effective the detection or prevention system.

  • Make the product G Í O Í D = C. "C" is the threshold or criticality index.
