6. How do you rate failure modes?
Use your customer's quotation grid, if available. If necessary, use a standardized grid or your own. Grids are also available in the FMEA study repositories (PFMEA from the Automotive Industry and Action Group).
For each failure mode, rate its Severity (S). Refer to internal and external effects for this assessment. The higher the value, the greater the effects for the user.
Rate the occurrence (O), or probability of this failure mode. The higher the value, the greater the probability of occurrence.
Rate the probability of non-detection (D). The higher the value, the less effective the detection or prevention system.
Make the product G Í O Í D = C. "C" is the threshold or criticality index.
