3. What lessons can we learn from the accident?

Analysis of the accident has identified the root causes. We now need to ensure that this type of accident does not happen again, by implementing a corrective action plan to address the root causes.

3.1 Write the investigation report

The head of the investigation team draws up a report based on all the information gathered. The report must be factual and as precise as possible, with diagrams, photos and a detailed description of events and causes.

All assumptions and hypotheses must be excluded from the report. However, it is possible that certain causes have been determined through deduction. In this case, the report should specify the logic that influenced this decision.
