The basic principle of a safety management system (SMS) is to be prepared for the eventuality of any accident.
The notion of safety is all too often associated with a situation where accidents are totally impossible. The Larousse dictionary even gives the following definition: "safety: a situation in which there is no danger to be feared". This definition is too restrictive, as it does not correspond to the management concept adopted by major industrial companies.
Numerous examples clearly show that accidents have happened when industrial players thought they were completely safe. Here are just a few examples from an extremely long list of disasters in the 20th century:
