1. Context

Feedback from experience (REX) is a regulatory requirement for high-risk industries, but potentially concerns all organizations seeking to improve their performance. On the face of it, its principle and purpose are simple: to learn from experience in order to improve safety levels. However, companies that have embarked on this approach may have encountered major difficulties in implementing it in practice, and have been relatively disappointed by the tangible results - which are sometimes difficult to assess - in relation to the efforts made. The risk would be that REX would be perceived as nothing more than a bureaucratic burden imposed from outside, whereas it remains a key process for improving safety.
