4. How do you send out the cards?
4.1 Who do I send the sheets to?
Send a copy of the form to your occupational physician. It will be archived in your medical file.
Inform workers of the existence of this sheet, and explain its content and purpose (e.g. during chemical risk training). Employees have access to the sheet on request.
You will give the form to the employee when he or she leaves the company, or if he or she is off work for at least 30 days as a result of a work-related accident or illness (or 3 months for any other reason).
The forms are made available to inspection officers during their visits.
In the event of the employee's death, his or her heirs can obtain a copy of the updated version of the file....
