Information to be controlled and maintained by an organization (3.1.4) and the medium on which it appears.

Note 1 to article: Documented information can be in any format, on any medium and from any source.

Note 2 to article: Documented information may relate to :

• environmental management system (3.1.2), including related processes (3.3.5) ;

• information created for the organization's operations (also known as documentation);

• evidence of the results obtained (also known as recordings).
