5. How to carry out a risk analysis and implement preventive actions using the Ishikawa diagram?
The principle is the same, but in this case, the effect is the defect or failure we want to prevent. Possible causes are identified in each category. Then, for the causes deemed most likely or most significant, we check that adequate means of control exist to prevent the occurrence of these causes, or we decide to put them in place.
From the list of causes, it's now time to build an action plan aimed at resolving each problem. Once the causes have been perfectly identified, it will be easier to determine the measures to be implemented to eliminate these problems, to evaluate their cost (material, time spent, resources consumed...) and their effectiveness. As we have just shown, the Ishikawa method is perfectly suited to managing the risks inherent in a project.
