7. Mistakes to avoid

7.1 Don't neglect the preliminary risk analysis phase

This phase largely determines the completeness of the scenarios analyzed as part of the risk study.

The method helps to structure thinking, but does not dispense with the need to use field investigation and creativity-brainstorming methods to identify the real causes behind defects. An obvious secondary cause can sometimes hide the main cause. In this case, you need to dig deeper to find the main cause.
