7. Mistakes to avoid

7.1 Don't confuse facts with opinions, judgments or interpretations.

Avoid, for example: "In my opinion", "Mr. Y neglects...", "Mr. X was late". The cause tree must be made up of precise, concise and verifiable facts (a state, an action).

7.2 Don't waste time creating a cause tree

Fact-finding should be carried out immediately after the accident. It can, however, be repeated at a later date to complete the information.
