2. Gather the facts
The quality of the analysis depends fundamentally on the information that the person in charge of the analysis is able to gather. It is therefore vital to carry out the survey as early as possible, and to check the veracity of the information gathered.
To ensure that nothing is overlooked, the investigation can be carried out at the scene of the accident, in the presence of all those involved and witnesses present at the time of the accident.
The record of facts will include witness statements, as well as the circumstances of the accident. This will involve :
collect concrete, precise and objective facts;
examine all aspects of the work situation;
to trace the causes as far back as possible, starting with the injury or pollution...
