 INTRODUCTION

Whether you're faced with a simple or complex problem, it's important not to limit yourself to the apparent causes of the problem, so as to be able to propose appropriate, lasting solutions. The way to do this is to identify the fundamental or root causes of the malfunction. The "5 Whys" method is an easy-to-use tool for quickly pinpointing these root causes.

Combined with an Ishikawa diagram, the "5 Whys?" tool will give you a more precise and structured view of the causes that led to the situation, especially in complex situations where some causes are not intuitive.
