Does ISO 9001:2015 require us to produce a quality manual?

NO. Having a quality manual was a requirement of ISO 9001 in its 2008 version, but disappears from this standard in its 2015 version.

When should the quality manual be reviewed?

As often as necessary, and whenever there is a major change in the organization of processes or the system, or when the quality policy is redefined if it is included in your quality manual.
