3. D2 – Defining and understanding the problem

3.1 The activities

  • Precise description of the malfunction (defect).

  • Define the conditions under which it appears.

  • Ensure that the "8D" team shares the same understanding of the problem.

  • Understand how the customer perceives the problem.

  • Try to reproduce the fault. Record measurements.

  • Identify the effects, characterize the scope of the problem.

  • Identify associated records (drawings, inspection sheet, report, complaint sheet, technical sheet, etc.).
