2. How often should the management review take place?

The standard does not impose any specific frequency, but specifies that the management review must take place at "planned intervals". It is therefore up to you to define the frequency of your management review.

Even if, in theory, it is not compulsory to carry out a management review once a year, this is the frequency most companies choose. You can also decide to hold more frequent reviews, or to spread the content of the management review over several meetings. Be careful, however, not to transform the management review into a simple reporting exercise; this is not its vocation, as it must retain a global and strategic dimension. What's more, data collection and preparation for the management review represent a considerable workload, which is incompatible with a high frequency of meetings.
